Dublin Pharmacist Yorkuri Admits Medication Error; Patient A, 82, Died in 2022
A Dublin-based pharmacist, Barinedum Yorkuri, admitted to supplying an incorrect blister pack of medications to an 82-year-old woman, patient A, at Cappagh Pharmacy in Finglas in April 2021, a PSI inquiry heard on Thursday. The medications were not prescribed to patient A and were not clinically appropriate.
Patient A became unwell after taking the medication and was hospitalized on two occasions, spending over a week there. She died in May 2022. The pharmacy implemented new procedures following the incident.
Yorkuri agreed to retake two Irish Institute of Pharmacy courses and not repeat the mistake. The committee agreed to Yorkuri's application for undertakings, making no findings against him, as it was an isolated error.
Yorkuri apologized to patient A's family, who accepted his apology. Barrister Susan Ahern, chairwoman of the committee, said the dispensing error “should not have occurred.” Yorkuri has had an unblemished record before and since the incident.
