The inquiry determined that a 2001 policy shift to move patients with learning disabilities and autism into community care lacked necessary investment, causing discharge delays, distress, and readmissions. A «closed culture» among staff discouraged reporting, and families feared complaining. Peer-on-peer abuse escalated dramatically, and seclusion was misused as punishment.

CCTV footage was crucial in revealing the truth of the abuse, which systems and structures were «wholly inadequate» to manage. The inquiry issued 106 recommendations, proposing reforms for «ineffective» external inspection regimes and serious governance failures within the Belfast Health and Social Care Trust. Kark emphasized immediate and rigorous implementation of these recommendations to prevent recurrence. The hospital is also central to the UK’s largest police investigation into alleged abuse, with prosecutions ongoing.
